I have set up a filter as follows...
---------------------------------------------
* Add to blacklist
* Mark for deletion
* All of the rules below are satisfied.
Rules: The body contains "inches"
The body contains "penis"
---------------------------------------------
I am now getting repeated emails containing...
VP-RX will take your sex life to new levels... Guaranteed!
Your penis will grow up to 3 inches
Your erections will be rock hard
Your sex drive will be supercharged
Your orgasms will be more intense
Your partner will be astounded
-----------------------------------------------
... mailwasher won't blacklist/mark for deletion....Why???

Try going into the "Raw Source View" by clicking the little icon with the magnifying glass which is over the message preview window. If you see something that looks like:
M<!H>A<!H>LE OR<!Y><!S>GA<!U>N ENLARG<!Y>EMENT
or
#111ur email address was subscribed using
Then the spammer has used some HTML tricks to hide his spam. Although the message will look fine in the Normal View mode, the filters work in the Raw View mode, so your words won't be seen.
There are some other possibilities, but these are a couple of the most often used techniques that I've seen.
